《健康123》LDL高、HDL低 引發動脈硬化的警訊
膽固醇是構成細胞膜、製造賀爾蒙、維生素的重要成分。人體內的膽固醇約70%-80%是由肝臟在體內製造;只有不到30%-20%是由外來食物攝取。在體內流動循環的膽固醇量，會由肝臟自動調節。當從食物中攝取過多膽固醇時，肝臟的製造量就會減少;若從食物中攝取較少膽固醇時，肝臟就會增加產量，供應身體所需。
其實，人體必需的膽固醇大部分都是由糖分、脂肪、蛋白質代謝(營養素在體內分解，合成新的物質的功能)製造出「乙酸(ethanoicacid)」作為原料，在肝臟中合成膽固醇來供應全身。
而「低密度脂蛋白(LDL)」就是負責將膽固醇送至全身及末梢組織的搬運船，其運輸的膽固醇就稱為「LDL-C(LDLcholes-terol)」。
因人體細胞表面有LDL的受容體(Recep-tor)，當LDL與此受容體結合後，就能進入細胞中，讓細胞利用LDL中的膽固醇(LDL-C)。然而，細胞只會攝取必要所需的膽固醇量，用不完的膽固醇就會露出在細胞表面。
另一方面，「高密度脂蛋白(HDL)」亦透過血液在全身循環，其功能是將組織利用過剩、露出在細胞膜表面的膽固醇拔除回收、運回肝臟，扮演著膽固醇清道夫的角色。
由此可知，並非HDL含的就是好膽固醇，LDL含的就是壞膽固醇，而是兩者功能不同:LDL是輸送膽固醇，輸送過剩時會造成動脈硬化，更甚時動脈硬化持續惡化，血管內容易形成血栓(血的凝塊)，甚至引發腦梗塞、心肌梗塞的危機。
HDL是負責膽固醇回收，可以預防動脈硬化的產生，與LDL作用相反，這就是好壞說法的根據。
而造成LDL上升，HDL下降的主要因素，包括:飲食過量、肥胖、菸酒、運動不足、壓力及女性停經等。（作者劉輝雄 京都府立醫科大學醫學博士）
